NT reinstall error

I am attempting to reinstall NT server on a machine. In
part one of the setup I delete all of the partitions and
create a new one and format it NTFS. The install completes
these steps and asks me to remove any floppy disk and cd-
rom disks and reboot for the installation to continue.

When the machine reboots i get an error missing
NToskernal.exe. Also when the machine reboots it appears
that the old boot.ini file is loaded because when the OS
chooser menu appears i get the old choices which should
not be there because i reformatted the drive.

has anyone seen this before

I have seen an old boot.ini come back when I did a quick format of the
drive instead of a full format. An incorrect boot.ini could also explain
the "missing" ntoskrnl.exe.
