Shiva Dial-Out Chooser and Control Panel Icon

I have completely uninstalled the Shiva Dial-Out Chooser
from 3 NT machines in my area via Add/Remove Programs, but
the icon still remains in the Control Panel.  I've
searched the registry for any references to this software -
- and there are none.

Can anyone tell me how I can figure out how this icon
keeps loading into the Control Panel when the program does
not exist?

Search out and delete the CPL for it in the %windir%\system32 directory.
