Make a mobile "wsus server"!
Install WSUS on you laptop for exemple, sync with msft. You will have
all fixes available for all of your clients...
Configure a GPO at your dentists on the W2K server that point to an
alias such as mobile-wuw.dentists.foo and updates all of the clients
with automatic updates

He mentioned that he had a laptop with XP Pro on it, so installing WSUS
isn't an option - although that was what I wanted to suggest as well.
There used to be a way that you could 'trick' XP into installing SUS, but
this isn't the case (as of yet) that I've seen for WSUS.
Perhaps you could take that server somewhere where there IS an Internet
connection, install WSUS, synchronize the content, etc., then bring the
server back to the office to distribute the updates.
If you do have a copy of Server, then I would install it on the laptop
(maybe you have a spare hard drive around for this?) and do as nmazurier
stated. Heck - if you have been to a MS training session where you received
180 day version of server, you could probably use that...
