Terminal Server Client Licenses

I have a customer who purchased 5 TS cal's for 5 Win 98
machines.  One of the machines had a hard drive failure.  
We replaced the hard drive, but it will only connect using
a temporary license.

According to KB article Q248430, there is a one time
permanent transfer of a TS cal, available by contacting
Microsoft by phone.  The article does not list a phone
number for the Terminal Services Licensing Customer
Service Centre.  Anybody know the number for Canada? is
there any cost?
