The SMTP que on my exchange 2003 is always full and the users are getting a
lot of notices telling them that their messages were delayed.  What is the
best way to tropuble shoot this issue?

Two things I would look at first.

1. Do all the message in the SMTP queue seem to be legitimate messages? In
other word has you Exchange machine been compromised.

2. I would do a close examination of your DNS.

Check DNS and make sure resolution works for all mail servers
nslookup is a good tool, ensure the suffix is correct for all Exch Servers
