Having a problem with a customer who recently had to swap out his Small
Business Server and thus his Exchange data.  Any help would be appreciated.

Since he was only using Exchange for Public Folder centralization (no user
mailboxes contained any signifigant data) the data migration was done by:

exported the Public Folders data to a PST file
recreated the Public Folders on the new system
imported the data from the PST

This process worked fine until I realized he had a custom Outlook Contacts
form created. I still have the old server and figured I would export/save
the custom form from that machine and "Publish" it on the new one.  But I
can't seem to figure this out..

any help or direction would be very much appreciated.

thanks,
-mike

If you can open up Outlook to access the old PF store, you should be able to
export the form out to an .oft file. Try posting in microsoft.public.outlook
for more expert help.

Thanks for the advice.  I will post to that newsgroup.
